We are the West Yorkshire Health and Care Partnership. We work with people and organisations across Bradford District and Craven, Calderdale, Kirklees, Leeds, and Wakefield. We help make decisions on health and care services (such as GPs or hospitals).We are an NHS organisation that helps people live long and healthy lives. The Government are writing a 10 Year Health Plan to fix the problems in the NHS (the National Health Service), and we are collecting people's feedback to help with this.To write our plan we need to find out what people think about NHS services in England. It is important that everybody can have a say about the NHS. For example, patients, carers, staff, and the public. This survey will help us understand what needs to change in the NHS. You do not need to answer every question.You can ignore some questions if you cannot or do not want to answer them.Any feedback we collect will be used to help us plan and pay for services in West Yorkshire. We will share your thoughts and feedback anonymously with the Government to help them with their 10 year plan. This will be done by the deadline of 14 February 2025.
