Survey of Stakeholders of the HomeFirst Programme

Introduction

The Health and Care Evaluation in Leeds have been asked to carry out a process evaluation of the HomeFirst programme of work.  As part of this we would like to have your opinions and thoughts about the programme of work and how it has been implemented.  This will feed into an evaluation of the programme which will feed into the planning for similar programmes of work that will be carried out in the future.

We're interested in your point of view because you have been identified as a stakeholder in the HomeFirst Programme.  HomeFirst is a transformation programme which is focussed on improving intermediate care in Leeds.  The programme is made up of five individual projects, and you may be more familiar with some of these than you are with the broader programme.  These projects are:

• Active Recovery at Home: which aims to develop a single, short term, home-based rehabilitation and recovery offer with the required capacity and approach to support more people at home, and to support people to achieve more independence outcomes through referral and triage, care delivery and discharge and systems and data.
• Enhanced Care at Home: a partnership which brings together health and care providers from across the City to deliver fast and effective care outside of a hospital setting for adults in Leeds. This will help people who may not need to go into hospital to stay at home where they are most comfortable and support others to return home more quickly after receiving care in hospital.
• Rehab and Recovery Beds: redesigning bed-based rehabilitation services in the community with the aims to deliver strength based and goals focused models of care for everyone in a community bed and to maximise the number of people who can return to their own home following a stay in a short-term bed.
• Transfers of care: aims to redesign how services connect and work together to improve the person’s journey through our system The goal is to create simple, quick, and effective transitions between services, and improve support for people who are discharged from hospital and their families and carers.
• System visibility and Active Leadership: which aims to support colleagues across Leeds to make evidence-based decisions to help with managing pressure on services, improve efficiency, and enable the most independent outcomes for patients. This will be delivered using technology and active leadership. The technology brings information together from different health and care services and presents it visually, alongside key insights, to help understand pressures on the services of Leeds. Active leadership means making sure the right people are reviewing the data and working collaboratively, to make data driven decisions and getting to the root cause of problems.
