You and Your GP - Feedback and concerns form If you wish to provide feedback or express a concern, your GP practice is the first point of contact. If you are not comfortable sharing your feedback directly with your practice, please use the form below to share your feedback or raise a concern. The more information you provide the more helpful it is, but please only share what you are comfortable sharing and please do not share person identifiable details or clinical details.

The information you provide will form part of our ongoing General Practice Quality Improvement work. We will take the information you share and liaise with local Primary Care Teams with the aim to improve GP Services and Patient Experience

Please note you will not receive further updates in terms of the information you share. However, please be assured that we will act on this information and we are working with Practices to improve Quality, Access and Experience for all our patients and communities

If you wish to make a complaint about your GP practice rather than give feedback, you should do this directly with your practice. The ICB can also submit a complaint to your practice on your behalf. More information is in the comments, concerns and complaints section of our website.
