We will share your experiences anonymously with your practice, including your Patient Participation Group (PPG), and use it to help us plan and improve services. To help us understand your visit, please provide us with as much detail as you are comfortable sharing. All questions are optional (apart from the first one) so feel free to skip any you don't want to answer.
