Survey Closes 20 Nov 2026

We want to hear your views on how your data is used in One Health and Care.

One Health and Care is a confidential shared care record that allows health and social care professionals directly involved in your care to view relevant information about you which is held by other parts of the NHS and social care.

This means that professionals can instantly access the information they need to make important decisions about your care - ensuring that you and your family receive safer, tailored care and treatment wherever you may need it.

One Health and Care includes GP practices and their patients across West Birmingham, the Black Country, Shropshire, Telford and Wrekin, and Staffordshire and Stoke-on-Trent.

As well as using data for your care, One Health and Care may also use anonymised data to help plan and improve health and care services.

We would like to hear your views on how anonymised data may be used for this purpose, and how much you currently know about One Health and Care and how it contributes to your care.

This survey will take roughly 5-10 minutes to complete, and it will close on Friday 20 November 2026.
